FIVE POINTS FAMILY HEALTH was started by Drs. Jane Chow, Roselynn Chuong, Jessica Fulton, Jennifer German and Patricia Smith in 2016. The doctors of FIVE POINTS FAMILY HEALTH completed their Family Medicine Residency training together at Women’s College Hospital in 2010, and have been practicing together since 2014.
